Position Overview:Responsible for accurate and timely dispensing of prescription
drugs to patients. Provide drug information to physicians,
nurses and patients. Plan, implement, and maintain procedures
for mixing, packaging, and labeling pharmaceuticals, according
to policy and legal requirements, to ensure quality, security,
and proper disposal.
Responsibilities & Duties:Essential functions include, but are not limited to:
Accept, validate, interpret and review quality of prescription
orders.
Processes prescription orders and enter information into pharmacy computer billing system.
Perform prescription assessment and verification, including DUR, formulary, etc.
Communicate and provide drug information and orders to physicians, nurses and patients.
Consult with other health care professionals to plan, monitor, review, and evaluate the quality and effectiveness of drugs and drug regimens, providing advice on drug applications and characteristics.
Provide information and advice regarding drug interactions, side effects, dosage and proper medication storage.
Analyze prescribing trends to monitor patient compliance and to prevent excessive usage or harmful interactions.
Reconcile drug therapy management or billing issues.
Formulary Management--assist with therapeutic Interchange suggestions or help obtain prior authorizations.
Plan, implement and maintain procedures for mixing, packaging, and labeling pharmaceuticals according to policy and legal requirements to ensure quality, security and proper disposal.
Maintain records such as pharmacy files, patient profiles, charge system files, inventories and controlled substance records.
Compound and dispense medications as prescribed by doctors and dentists by calculating, weighing, measuring and mixing ingredients or oversee these activities.
Prepare sterile solutions and infusions for use in patients' homes.
Operates pharmacy in accordance with state and federal regulations while supervising daily activities of pharmacy technician.
Other duties as assigned.
Education:Graduate of an accredited college and licensed as a Registered Pharmacist in the state of Ohio.
Experience/Knowledge/Skills/Physical Requirements:
• Previous retail pharmacy experience preferred. Hospice patient experience a plus.
• Previous sterile IV preparation preferred.
• Must be self-motivated, outgoing, with an excellent customer service attitude.
• Excellent interpersonal and organizational skills (a team player).
• Normal office/clerical motor skills plus ability to stand 4 hours at a time and lift 10 pounds.
